    I cannot access my account due to it saying account autosuspended. Please advise. Thank you for your help.

    You have been suspended due to the login fields in your form. You are asking for a Godaddy login. Please note that such forms are against our Terms of Use. Please refer to our terms of use here https://www.jotform.com/help/8-Terms-of-Use.

    I have reactivated your account, but I have deleted the said fields from the form. Please refrain from adding the same fields again.
