Yes, you can create custom domain forms. But, this feature is only available for JotForm Enterprise users. I would suggest upgrading your account to JotForm Enterprise if you wish to use custom domain feature.

Hi! 

When you upgrade to Enterprise, we provide a stand-alone server for your organization. Your forms and data will be moved to that server and you will be able to set your own domain for your forms. It can be www.mydomain.it/123456789 (example form link). 

We also offer direct support contact, service level agreements and things of that nature to enterprise users.  There are user limits for Enterprise, minimum number of seats to buy is 3:
