I am using this form for people to sign up for classes. When they choose a date, it is not registering on the form that I get or the customer gets.

    Hi ,

    I cloned your form and found out that in your email notification your date is set to {dateselect11}. 

    To go to notification please follow steps below.

    1. Open your form to edit.

    2.Click Setup & Embed > Email Alerts >Thrive photography photo class.

    Date(select only one)  = {dateselect11} , please change it to {dateselect}.

    It should look like this.

    Then click finish.
