    We are performing a Jotform Anamnesis survey as part of our dental assessment and interoral scanning processes in our stores in Manchester and London.


    Can you please take that blocker out for the iPad located in Manchester

    It seems that you have entered a Private IP Address. Please check any of these sites using the device that was having an issue and give us the Public IP Address.
