Why is our account suspended?

    My account is suspended for too many failed password attempts.  I changed the password.  How do I get logged back in?

    I see that your account thuff82 is suspended due to the logo on your form which seems to be having a copyright. Online Form Builders, including Jotform, are sometimes being used for identity theft and we go to great lengths to prevent this. 

    Our phishing detector will automatically flag the forms found and suspend the account or this can be manually performed by our Staff. If your purpose is not to collect these data from the general public, but from your own customers, then you need to comply with two things.

    1. Add the Terms & Conditions Widget (or the Short Scrollable Terms Widget) for your users to read and agree.

    2. Add a Signature Widget so your users can sign and agree to provide such sensitive information.

    However, we do not allow uploads of any sensitive personal information like passports, social security cards, driver's licenses, credit/debit cards and statements, bank statements, voided cheques, birth/marriage certificates, visas and photo IDs.

    As for the copyright logo, in order to reactivate your account, kindly provide us the following information:

    1. The URL where your form is used.

    2. Your website or proof of affiliation to a legitimate institution.

    3. Share with us a document that will prove that you're authorized to use the said logos.

    We also need you to change your registered email to a corporate one to avoid getting flagged again by our phishing detector.

    Send these proofs to reviewers@jotform.com with Thread #1922141 as your subject. We will let you know about the status of your account after a thorough review of your proofs.

    Or, we can just remove the offending fields and logos so we can lift the suspension on your account.

    I am currently experiencing the same issue. Can you remove whatever is improper for my agency.

    I am fine with just removing the logos. Also, I am an educator and am
    trying to purchase the bronze education plan, but I haven't been able to
    get a response from jotform. I am in urgent need of the subscription. Thank

    I removed the logo from your form and activated your account back to good status.
