How do I edit/change the header of our form submissions email notification template?

    Also how do I remove "Ultrabrand Logistics" from our form submissions?  This is our old web developer that we are no longer in contact with.

    Please let me know if you need any more information.

    It looks like all our forms were shared with us from old web designer so I cannot edit them or the emails attached to them.

    Is that correct?

    Hi Sue,

    You were right. As checked, the forms are only shared to you so you won't have access to edit it. You may contact the owner of that form to either edit them for you or let them make a request on this link to move that form to your account permanently.

    You can also clone that form by following this guide: How-to-Clone-an-Existing-Form-from-a-URL. However, this will only copy the form's design, conditions, and settings. It will not include its existing submissions. The Form ID will be changed also.

    So if they request to move the forms to our account permanently there will not be any change to the links/website/email/Form ID used by our website? 

    Also if the forms are moved, will they be under our upgraded HIPAA Compliant folders?

    Also if the forms are moved, will they be under our upgraded HIPAA Compliant folders? — Sorry, but we can't move the forms to a HIPAA account. I would suggest that you clone the form inside HIPAA account.

    So since I updated to Silver & upgraded to HIPAA, I cannot open the inbox for our forms that are in the shared folders.  Looks like it is trying to open an inbox under hipaa.jotform.com.  Are my forms working?  Can I still get to the inbox?  When I asked about upgrading, there was nothing about any forms or URLs changing.
