Personalize error message

    How do I edit this personalized error message when forms have been deactivated? (See image)

    I had deactivated a form, and when someone clicked on it, they got this message- problem is, that’s not my information! 

    Maybe there’s a glitch because she was the original creator of the form, and I imported a copy?? How do I fix this?!
