Conditional form fields based on selection

Hello. I've got a form that I'm using to create part requests for 1500 stores. What I would like to do is when the user selects the Store Name and City, it auto populates the address and phone number for that specific store. I have all of this data in a separate excel file, so my question may be twofold as I'm wondering if I can import this file into Jotform?

    Thank you!!

    Hi!

We have a Spreadsheet to Form widget. It allows to attach an excel file to a form. The form should be then populated based on unique code user enters. That code you could generate by combining values of Store Name and City. Form Calculation widget should be of help to do that.

Please review the help guides I'm linking below:

We will be glad to assist if you need more help, just let us know.

The spreadsheet to from widget seems to be limited to specific field names.

I have changed my fields to have a Dropdown of "City" and then a Dropdown of "Account".

    I'd like to:

When the user selects the "City", it filters to show only the "Account"(s) in that city. (This isn't a necessity but would be nice).

More importantly, once the user selects the "City" and "Account" the "Address", "Province", "Postal Code" and "Phone Number" auto populate.

Here is a screenshot example of the master excel file:

    What are my options, here?
