How can I create two click through boxes that go to one survey or another?

    I need two different forms attached to the same link. I need the first page to show two text boxes, one that says United States and one that says Canada. I need the United States box to go to a specific form and the Canada box to go to another form.

    I need to show two different forms to two different regions. I need US consumers to be able to choose a "US" option and be taken to one form; I need Canadian consumers to be able to choose a "Canada" option and be taken to a different form.

    You can use a combination of single choice field and text fields in the form.

    Create similar text fields for both US and Canada customers. Add text and insert the form links (one for US and the other for Canada) as shown below.

    Now create conditions to show the related texts based on the choice.

    This looks like a good option, but I'm concerned a consumer will choose their region then try to submit the form, which won't take them to the survey. Any ideas on how to avoid that?

    You can add "Get Visitor Location" widget to your form and strip the country part using "Substring widget" then make a conditional logic to hide the fields if the country is not equal to the selection.
