Is there a widget that can do that attached image?

    I'm sorry, but I don't think we have a widget that can do the attached image. The closest one we have is the payment tool with Quantity option enabled.

    Please try that first with our PO tool so you can set up the items without any integration required such as Paypal.

    The only thing that is not possible in the payment set up is the product description.

    Do you have the website where we can check the overall form structure of the attached image? We can possibly send a request to our widget team for this.
