Having Trouble prepopulating One form value to another

I have 2 forms. One that a person initially fillout and then will get a link to the next form prepopulated. I have gotten alomst everything to work. The only thing I can't figure out is the AM and PM in the time. everytime I put the formula i, and certain prepopulated areas disappear. Here are the links to the forms.
