Why is my account is suspended?

I cannot log in to my account. 

Your account was suspended. The form 91982877784885 is collecting Passwords from the users which are against our Terms of Use.

Phishing. You agree that Company may terminate your JotForm Service immediately if a form's purpose is found to be deceptively obtaining: sensitive credit card information, social security numbers, user login credentials, or other sensitive personal information. All these activities are considered as Phishing and any account along with the violating forms will be suspended immediately.

I have now removed the password fields and re-activated your account. Please refrain from doing this as it is against our terms of use.

Thankyou for response, I will have to find another way for new members to set up their logon to our website. The password field that was on this form, was for new members to set up their logon online for the website.
