How can I create a replica of a PDF warranty form?

We have a warranty form at the moment (as attached in PDF form) would you be able to create a replica in jot form or if not what is the best template to use?

Since the PDF form or document provided in your screenshot is having boxes for the fields which could cause issues if it is imported. However, it is easy to create a form from scratch using the form builder by drag and drop method. 

You may also check the form templates that are shared by our other users to clone it to your account.
