PDF fillable form doesn't show all the fields sequentially

    when publishing file as "fillable pdf" not all rows (questions) appear at the right place in pdf. what's wrong?

    I notice two Form field is not synched to the sequence one is email field and the other one is the Date of PT.

    Could you please drag these two fields and move to the right place based on the online version.
