el formulario cambio en diferentes pantallas

Buenas tardes , al crear el formulario y revisarlo desde diferentes pantallas se ve diferente leyen un post ya todos los formularios son responsive.

como podria bloquear lo cuadros de texto para que no se muevan

Please note that our forms are mobile responsive by default. When the size of the screen is changed the form will automatically adjust to the new width. I inspected your form and I saw that you positioned multiple form fields in the same row. You also have a lot of conditions that are set to show or hide some of those fields. Considering that the different number of fields will be shown in the row, based on the selected options, it is not possible to align the fields properly.

You could only show them like they are set inside of the Form Builder if you delete your conditions. 

Instead of positioning multiple fields in the same row, maybe you can use Configurable List widget or a basic Input Table field.
