Incomplete values error message

Olá, 

apos preencher os campos obrigatórios e enviar o formulario, recebemos a tela de Erro "Incomplete Values". (veja o print)

Quando retornamos aparece a tela de agradecimento, o formulario é enviado normalmente, não havia este problema, como corrigir?
