not able to publish form on google site

Hello, 

Form #62264568587167  used to work just fine on my google site until now and all of a sudden , this form is not showing up.

I  used the following method to republish the form but its still not working.

page where the form is supposed to appear just shows blank.

this is the site, where in the middle, jotform displays store items for  parents to place an order.  But as you can see, it is blank. 

Would you be able to help me fix this ? This is for my school which is going to open soon and parents will need access to the form to purchase school supplies..

I checked the website and that embedding method and I was able to replicate the problem.

So it might be an issue with embedding method.

Could you please try inserting your form by using Embed option:

And then just insert your form's URL

I just tested on my test Google website, it worked fine.

Please advise how it worked for you.
