How can i create multiple sign-ons (Hazard Assessment Form, pre-work assessment)

Hi Team,

I am in the process of creating a JSA (job site hazard assessment) form.

The purpose of the form is so that a team are able to assess the job site, write down hazards, and go through required safety precautions before commencing work.

Query 1:Every person on the team must sign on to the form, the form also needs to be emailed or texted to each person that signs on. (I have information of every employee so I can pre-load their email addresses and mobile numbers into the form if required.)

Do you know a widget that can do this for me?

Query 2: I would like to add images to the form, such as an image of a risk assessment matrix.

How do i add images?

Do you want to build two Forms, in the First Form you want the customer to enter the basic details(email, name, mobile Number) then when the user submits the Form it should email the user to fill the rest of the second Form and pre-populate the basic details in the second Form?
