Can I add checkbox to save and return form?

Another question – can I add a single choice question to the save and return? I’d like to determine the region the person lives in. When I tried to add it the check off boxes don’t show up.

Yes, you can add a single choice field on your form. In fact, all types of fields can be added to the form. I've tested it and it works fine.
