May we know the website URL where you've placed your form so we can inspect it here?

    Thank you!

I have checked your website and it seems that you are using a Jotform direct link. If you need to maintain your website header, you will need to embed the form in your html page or create an html page with the embedded form instead of using a direct link.

To embed the form in your html page, you can either use the IFRAME method or the embed script which is the easiest to use. By selecting and clicking on of the icon, it will go to the next page that contain the embed form script.

     Copy the Jotform embed script.

Insert/Paste it in your html web page.

Click here to download an example of the HTML page which is based on your contact page wherein I embedded the form using IFRAME method. By removing the under construction image in your contact page and replacing it with the embed script, I was able to achieve the result below.

A detailed instruction on how to embed the form in your website  using embed script can be found on this link. For IFRAME method, please click here.
