Hi, do you have a form I can use to create a master file of options (and details for that option) that I check off options from then creates a simplified form for a client's plan?

You can use the Show/Hide Field condition so that another field will appear or hide based on the options selected from a Checkbox field.

choices (eg 15 choices) I can choose from then creates a form with a final
list of items chosen (eg 3 items finally chosen).
Can you provide a link that demonstrates this need?

We are sorry that we did not understand your requirement correctly. For the list of options you may choose the single or multiple choice fields and add a condition to show/hide the final options based on the selection.
