Problem with layout on my enrollment form

    Hi!

    I have been working on building an enrollment form most of this week for my after-school program. It's basically done, but I am having a problem with the layout that I can't seem to iron out. Everything looks fine on my phone via the app, but on my computer, when I go to Preview Form, the label text is pushed to the left margin and all bunched up.

    Also, as of this morning, when in Preview Form, all of the boxes are populated with things like "textbook_sample12".  I attached a screen shot.  I am on a Mac and using the Safari browser.

    Please help.  I am hoping to get this up and running today and open up my fall registration.

    There seems to be a conflict between the form's layout and the field's layout. I would suggest to reset the layout by following the steps below.

    On one of your field's properties, can you please try to toggle the field's Label Alignment to Left then Top, make sure the Set as form default checkbox is checked.

    Then on the Form Designer, Styles tab, toggle Label Alignment as well to Left then Top, and change the Label Width to anything then back to 600, and click on Save at the bottom?

    As for the autofill, I wasn't able to replicate it. Can you please try to clear your browser cache.
