How do I create a pop up in single card form?

hi there, how do i create a pop up for my single card questionnaire?

My understanding is that you want to embed the form to your webpage as a pop-up. If yes, then please refer to this guide: How-to-Open-Your-Form-in-a-Popup-Window.

If you want to put a placeholder text in the field, then you can do so in the Properties > Advanced tab. Here's a screenshot:
