So, I have created a registration form for at least 4 kids/registration because they get a lower rate if they register as a group. BUT... the problem lies when I go to print reports. Because there was only one submission I have to go search out other players. Is there a way to print reports based on how many people register for Initiation 1st year, Initiation 2nd year, Novice 1st year, Novice 2nd year? And know which player has registered for what camp?
I think I have figured out the quantity of registrations/form it's now figuring out how to set up the form so I can have a record for how many kids register for each camp.
Thank you,

You may want to use the Configurable List widget instead to add multiple entries of Skaters in your form.
Please check this demo form https://www.jotform.com/92296270993974
Let us know if this method will not work for you.

I did check the form which my colleague shared with you. Custom css code is injected in the widget to display the set of configurable list widget fields. Please check the screenshot below:
I would suggest you to please clone my colleague's form in your account. The following guide should help you in form cloning: https://www.jotform.com/help/42-How-to-Clone-an-Existing-Form-from-a-URL
Do get back to us if you want to implement this in your own form and we will do the needful.
We will wait for your response.
