Problem with submission.

    Hello,

I filled out the form (see screen shot attached).

    I clicked on the options for submission 2D, 3D & Time-based work

Time-based work came up & I moved through the application
thinking that once I hit submit for that area, the 2D & #D options
for uploading would appear as separate accessible categories.

Instead the entire form was submitted & I have yet to upload other
work. There is no way to edit or retrieve it.

    CAN YOU PLEASE HELP???

    thank you,

    Sondra

I also tried to attach 2 screen shots, but this “form” is only taking one.

Please note that JotForm is an online Form Builder. The form you submitted was created by one of our users.

If you did not fill all fields you wanted, I can only suggest you to report the issue to the form owner, by replying to the email you reived after you submitted the form (email from your screenshot).

Or you can fill and submit the form again.

As my colleague explained. We cannot delete the data of the other form owner. Please contact the form owner if you need such action.

I realize what you are saying, but I accessed this directly from YOUR site.
How do I contact the owner if there is no reference of who that owner may
    be?

Users are creating forms in our form builder (site), but we cannot control what they do with it.

In the email you received when you submitted it, there is a reply to email address, so if you just reply to email from your Inbox, you can contact the related person.
