Is there any way to create a form that tests a users eligibility for a program

Is there any way to create a form that tests a users eligibility for a program? For example, if they wanted to enter into a program but that program has income guidelines, how would I create a form that would let them know if they are eligible and if so for them to contact us. 

You can use conditional logic to hide/display fields if they are eligible for the program.
