I created 5 forms last week and today I need to use them to send out the registration forms and they are nowhere to be found. Please help!

My account looks to suspended. Not for sure why! Please help!

Your account is suspended because the forms you created are used to collect Driver's License using a File Upload field and that is against our Terms of Use.
If you want to collect Driver's License, you need to use a Short Text Entry (Text Box) field. You will also need to add the Terms & Conditions and Signature widgets to your form, for the users to confirm/allow/agree that you are collecting the information for a purpose.

We can only lift the suspension of your account if you agree with our terms and allow us to delete the offending fields.

I saw that my colleague re-activated your account on this thread - https://www.jotform.com/answers/1920473
However, I had to suspend it again because you did not agree with our Terms.
We cannot allow you to delete the fields. We have to delete them but only after you agree with our terms. So, please let us know if you agree and we will re-activate your account after we delete the fields.

Hi Shawnika!
We have now deleted the guilty fields and your account is active once again. As stated by my colleague above
