Account suspended

Hi. My account has been suspended again as I was creating a new form. I have no idea what I have done wrong. I would like to keep the account. Could you help.

The reason your account has been suspended is due to your GCC Free Assessment Form has been detected for phishing activities because you're collecting Login and Password credentials. 

 Such forms are considered as phishing forms and associated accounts will be automatically suspended. Please refer to our terms of usage for more information on phishing. We have to delete them but only after you agree with our terms. So, please let us know if you agree and we will re-activate your account after we delete the fields.
