I created a format and paste the code in my footer, but the format appears in all the pages in my website.  Is there any way to appear only in one page?

Probably, all the pages of your site use the same footer. Try embedding the form somewhere in the body of the page: https://www.jotform.com/help/34-Embedding-a-Form-to-a-Web-Page
