I would like to make an "if" function that sends the data to another excel spread sheet. I noticed that we have a few forms that aren't being used that I could modify to fit my needs. In general the form we use is for safety observations that go to a public excel sheet. I want to make an option to submit something to a separate form that only I have access to.

So if they click option 4 (for example) it will open a separate comment box and then submit will send it to a completely different form.

I am not sure if I understood your question correctly. To exactly which Excel Spreadsheet are you referring to? Google Sheet integration maybe? The form can only be integrated with one Sheet, so it is not possible to send the submitted data to multiple Sheets.

With conditional logic, you can show/ hide form fields. If you want to send the data to a separate Sheet you will have to use multiple forms. For example, you can embed your other form with a comment box to the form you provided with the iFrame Embed widget and set the condition to show it when option 4 is selected.

Inside of this guide, it is explained how to create a multiple payment form using several forms. You can use the same approach to create your form.
