Sorry, I didnt realise the form I copied in would disappear.

Basically, I need a form with sections for info to go across the page:

Name    Address      Tel No      Dates From    Dates To    Reason for leaving

    [ ][        ][              ][             ][                 ][              ][                             ]

Next to the list needs to be a tick box, then underneath needs to have a button to 'add more' lines or 'remove' selected.

    I hope this makes sense.

Hi there, it's possible to " Shrink " fields so that they are horizontal instead of vertical though in doing so you may need to adjust the form width and sizing of the fields to make them fit better.

As far as the Tickbox goes unfortunately we don't have an exact feature to create the form just like that adding more automatically and the checkbox input is possible but it would require the form's source and some additional programming to reach that extent.

Something that you could do to achieve this though is to set it up with Textboxes and Conditional Logic to make it do this as needed. However, if you would like I would be more then willing to submit your idea into our Features Request List if you are looking for something more advanced in this area.

Please see this form demo to get an idea of how this would work. Then you can additionally use some custom injected css to close the gaps if you want.
