Not receiving email notifications

Hello My Name Is Sharon Riley.  I work for AJL's Wings Of Hope.  We are a Food Program Sponsor in Desoto, Texas.   Yearly our Day Care Home Providers take the TDA Educational Civil Rights Exam On-Line through Square meals .org.  Once completed we get a copy  emailed to us through Jot Form.  I have two Providers  (Janice Spears & Carlanda Reeves) that have taken the exam and nothing has been email to their email address or ours.

Is there any way to retrieve this information..
