Hello,
For some reason when I duplicate my form it is not duplicating properly.
How do I fix this?

When you duplicate/clone a form, if you have any integration/widget that requires any key/token/API, it would need to be reintegrated with a fresh login.
Also, when you clone a form, you will need to ensure that you are cloning to the correct form layout i.e. if you have a Classic form, you will need to clone that to a Classic form and not a Card form.
Upon checking your form, I could see that you have figured it out, right?
