Unable to receive completed forms

    IM UNABLE TO RECEIVE COMPLETED FORMS

    Upon checking your account, I could see only one form - "New Patient Questionnaire" and so far there aren't any submissions for this form.

    On further checking your form, I found that there is no Submit button, which will not allow users to submit the form and hence register the submissions. Only then emails will be triggered to the setup email addresses.
