Form Not Uploading Properly (Fields left out btw template and code)

Please can you check the code below and let me know why this Sales Tax Id field is not showing up?

This is what it looks like in Squarespace after code (missing the Sales Tax info which is needed because wholesale buyers do not pay taxes and this is code given to corp/biz)

Your embedded form is displayed correctly, but I see a condition is applied to this field, so it will be only shown if associated condition is met. 

Please check if you need to adjust or delete that condition.
