I want to use your service for my gym waivers. if a prospect enters my club now, they sign a waiver (physical form). God forbid something happens, I can refer to that form with the prospects signature on it (they sign right under the release of liability). I'd like to create a form my prospects can sign digitally, then have a complete record of it in a .pdf. When I did a test submission (using the form I attached) I got emailed the field information, but nothing else. In other words, I get the signature, but there is no frame of reference for what they're signing. Is there a way to get the form sent to me exactly like the form looks (excepting the field information will be populated)?Page URL:

Unfortunately Free Text HTML will not be automatically included on the form notification. You have to manually copy and paste them there. To edit your form notification, simply click Setup & Embed > Email Alerts > open Notifications. You can modify everything there and reposition every form field tags found on the right side bar of the notification window.
