My form has been disabled and I can’t seem to log on

    Hi Jotform, 

    it seems that the form i created a couple of days ago is gone. I click the link and it leads me to a page that says “Form not Found. This form is disabled.” 

    I tried entering my credentials using google account but it doesn’t push through. I changed my password and tried logging in bu it just displays : log in failed - SUSPENDED” 

    Hoping you can help me out on this.

    Your account is suspended since the form found in your account was flagged under copyright logo scam.

    We understand this might not be the case with your form, yet we had to act according to the general rule to be in compliance with our Terms: Free accounts with non-corporate emails showing suspicious forms must be suspended immediately. 

    In order to reactivate your account, kindly provide us the following information:

    1. The URL where your forms are used.

    2. Your website or proof of affiliation to a legitimate institution.

    3. Share with us a document that will prove that you're authorized to use the said logos.

    We also need you to change your registered email to a corporate one to avoid getting flagged again by our phishing detector.

    Send these proofs to reviewers@jotform.com with Thread #1936130 as your subject. We will let you know about the status of your account after a thorough review of your proofs.

    Or, we can just remove the logos and lift the suspension on your account.
