How can I create a form for details on education and work experience?

Hi 

I am struggling to create a form that I can send to my clients and ask for their qualification and work experience 

Eg

Qualification 

- Type of graduation 

- university and college 

- country 

- dates from and too 

And then they can have an option to" add " if they have more qualifications 

Same thing for work experience
