Why do my forms keep saying they are disabled?

Why do my forms keep saying they are disabled?  In the settings, they show they are enabled.  I tried to go to a different tab and get a screenshot but now it says my account is suspended.   Why?!?!?!?

Your account and form was auto suspended by the Phishing detector because your form was flagged as phishing form due to the following fields on your form.

For now, I have reactivated your account and form again. 

I suggest you remove those mentioned fields on the form and never use them again. This will avoid the Phishing system suspending the account and form/s again.

If you need collect data on social accounts, you may use widgets instead.
