How to create a Quiz Form?

i am creating a quiz with different outcome pages. 

how can i accomplish this?

the issue is how do i get different results based on answers.

You can just open the form through the app and select the correct answers. Once the user submits the form with the answers, the total score and percentage will be calculated automatically based on the correct option you given.
