Why my account is suspended?

Hi,
We created a form for our employees to provide us their company email and skype user and password. Now, we can't access our jotform account.

Your account was suspended because your form 92336339098466 is collecting Passwords from the users which are against our Terms of Use.

Phishing. You agree that Company may terminate your JotForm Service immediately if a form's purpose is found to be deceptively obtaining: sensitive credit card information, social security numbers, user login credentials, or other sensitive personal information. All these activities are considered as Phishing and any account along with the violating forms will be suspended immediately.

I have now removed the password field and re-activated your account. Please refrain from doing this as it is against our terms of use.
