Login - AUTOSUSPENDED

Hi I got an  Login failed - AUTOSUSPENDED message.  

Could you advise please?

Your account is autosuspened by our Phishing system because your forms are used to collect passport number, which can be against our Terms of Use. 

If you want to collect Passport number, you need to add Terms & Conditions and Signature widgets to your forms, for the users to confirm/allow/agree that you are collecting the information for a purpose.
