Why my account is suspended?

Hi, recently I created my account and created a form, but now I can't access to the data. I restart the password but It didn't solve the problem.

I have a question. Does this widgets has any specific requirements? For example, this form is for an event on Universidad Sergio Arboleda, and the ID is requiered to give access to the University for those who are not related with the institution. I added a disclaimer according to Colombian Laws and a Terms and Conditions Acceptance, but here in Colombia is not regular to use signatures for this forms, and many people left form if they have to do something different to type and click on terms.

There is no further requirement other than linking the URL of the web page where the terms and conditions are located. If you want to display them in the form, you could use the "Short Scrollable Terms" widget instead: https://widgets.jotform.com/widget/short_scrollable_terms

The signature is something that we require to make sure that the user agrees to provide the sensitive information, so the form does not get suspended by our phishing detector system.
