I cannot log into my account

    Your account has been suspended due to the fields asking for driver's license and password, kindly note that you can ask for driver's license numbers as long as you have a signature and terms fields, this way your users agree to provide such information. 

    I noticed that you have already a signature field on your form, here is a widget that will help you adding the terms:

    Regarding the password field, unfortunately, it's not possible to ask for such information as it goes against our terms. In order to reactivate your account I went ahead and deleted the password field, please kindly check and let us know if you have questions.
