My form is not working

    i tried logging into my account using the email above and it says suspended.  why???  i need for my form to work.  the page is just blank when i click on my forms:

    You have forms that collect SSN details which are against our Terms of usage due to which your account had been suspended.

    If your purpose is not to collect SSN from the general public, but from your own customers; and if your business is among the list above and you comply with recommendations below, then we can make the exception:

    1. Add Terms of Use widget (for your users to read and agree). This will surely make your forms not to be considered as guilty of phishing activities

    2. Add a Signature widget where your user signs and agree to provide sensitive information.

    3. If possible, change the email address on your profile to one that shows your business domain rather than the public email address you currently show.

    We can only ask for consideration to reactivate your account and forms after you agree and comply with the terms.

    Oh ok.  No worries, I can remove that field...or you can.  We dont have to have it online.  Can you remove it from the form and make the account live again please?
