I had this issue happen a few months ago but I am no where near the limit of my plan and yet I am being told that I need to upgrade my service. Can someone please assist in this manner.

I have just checked your account and noticed that in fact, you have not reached any of the limits of your account; however, you're close to reach the forms and upload space:
You can also check that info following this guide: https://www.jotform.com/help/408-Understanding-Your-Account-Usage-and-Limits
You may receive alerts about these limits, but an upgrade is not currently required, your account and forms should be working as expected as far as I can see.
If you continue experiencing the issue, let us know.

This screenshot is what users are currently getting that are trying to use the form. If an upgrade is not currently required can you please explain why no one can use the form. Like I mentioned earlier this was a problem I had a few months ago and it was easily fixed as an error on JotForm

I have now cleared it and your form is displayed on web page. Let us know if you need more help.
