How can I create a order form?

Hi guys I am looking to create a order form that is filled out by our call centers and sent to the customer to pay the order on there email or with a link on the phones via text message so for example: We are a Home Security Company so the process would be the representative talking to the customer creating there order then sending that order to the customer and giving them a activation cost for what there system thats it. Then on the customer end they would receive a email and pay the activation cost with a summary of there order.
