Missing Form in the account

Dear Team,

I create a nice form only is this not linked to my account.

How can I see the results of this form?

It seems the Form was created from a guest account. I could not verify the account annebrouwers and the guest account so I cannot move the Form to your account. Since both were created from two different locations one from Newzeland and another from the Netherlands.
