Adding additional information for when data is captured by phone

    OK so I have a number of templates we send to customers to complete with their data.

    I want to add a number of questions if a staff member competes the data on behalf of a customer that phones in to the company. What is the best way to do this?

    I want to have the URL to send this to the customer as current, but when a staff member completes it by phone that additional questions such as the staff member name, date and time of the call would be recorded.

    I do not want to have these questions visible to the customer where we send out the form directly just when a staff member completes it.

    You can just add a single choice field, where the user will have to choose, whether he is your client or from your staff. If the user is from your staff, the additional fields will be displayed.

    You can require a code (password) for these additional fields. So, your customers will not be able to see the fields, even if they choose the option that they are from your staff.
