Can you have form fields populate info based on a spreadsheet or contact list? Such as if John Doe was sent an email can the Name and Email Address fields be pre-populated with the info submitted through a contact list or spreadsheet, that was uploaded to Jotform or an email application such as campaign monitor?

Hi @mhartprograde,
We do have a widget that you can add to your Form called Spreadsheet to Form that might come close to your requirement.
The Spreadsheet to Form Widget will allow you to upload a spreadsheet to your form and use the values you have on the spreadsheet to pre-populate the form. You will use a unique code present on each row of your spreadsheet. You can access that specific row when it matches the unique code entered on the form.

Another option is to prepopulate fields vias URL parameters.
